What should a service request form include?
The form should collect enough information to triage and respond without asking the requester to design the job.

The short answer
A service request form should capture requester contact, customer or property, exact location, plain-language issue, impact, when it started, access or preferred timing, relevant attachments and required privacy or contact consent. Operations should add priority, responsibility, scope and assignment after review.
Use conditional questions
Show access, safety or equipment questions only when relevant. A short first screen improves completion while conditional fields preserve the detail needed for specific services.
Protect public forms
Use validation, rate limiting, bot and abuse controls, reasonable payload limits and monitoring. Do not expose internal customer, job or staff information in a public submission flow.

- Ask who is reporting, where the issue is and how to make contact.
- Capture symptom, impact, start time and useful attachments.
- Collect access and preferred timing without promising a slot.
- Triage priority, responsibility and work scope internally.
